He could be heavily fined if he decides to sit while healthy, and the Spurs could pursue legal action since it's a breach of contract.

He could say he will have surgery, and I don't know a way around that, but sitting out an entire season would make him look REALLY bad.

He can't take the sit out threat very far, as the league would likely consider that a season that doesn't count against his contract. That said, how much of a headache do the Spurs want to deal with? They need to trade him somewhere. Even if it's not LA, the presence of Lebron should prevent another Paul George style change of heart, so it will be a rental.

He would most likely end his career temporarily by deciding to retire - then go play overseas for the next year and come back next year after the contract has expired - that's a risky gambit and quite frankly - I think the League should go after him legally - They can't let players pull that kind of crap
