Here's one I probably wouldn't have known to this day if not for my friend's wife. The guy that played Wesley in Princess Bride was the doctor in the first and last Saw movies. It had probably been close to 15 years since the last time I had watched The Princess Bride and the first time I saw Saw. The OP mentioned voices as a giveaway well she said she went to see it in theatre and you hear his voice before you see him because the lights are out. She said as soon as she heard his voice she knew it was him and actually started laughing. Not really sure what would be funny about that but whatever.
