Since SMU Football got hit with the death penalty for 1987, there have only been 2 recorded instances of the NCAA enforcing it. One was against Morehouse Men's Soccer and the other against MacMurray College Men's Tennis, both in the 2000s. Prior to SMU, Kentucky and USL (now Louisiana) both got hit with it for their Men's Basketball programs.
