How do you enforce a penalty from tge middle of a play? Either that penalty should be from after the turnover or from the start of the play. This way doesn't make sense

Because it wasn't just the middle of the play, the ball would have been turned over at the fumble if not for the penalty. Thus, the penalty is applied before the turnover.
Every other penalty that happens during a play (non dead ball) you either take the play and decline the penalty, or take the penalty and decline the play. Some will even let you do both, you can take the play and have the penalty still applied at the end of the play. I don't believe I've ever before seen a penalty applied from the middle of the play.
Again he fumbled during the play. If you apply a penalty at the end of the play it has to include the turnover. That's part of the play.
