I worked for a company that used first 6 of last name *then* first initial (for some damn reason). We had a guy whose email addy was dicksor.

Used to have one where they had first 3 of last name then first 3 of first name and sequential numbering, if needed, to break a tie.

Barbara Gayson ended up with the worst possible user name under that scenario.
