It shouldn't happen...As a Chef, there is NO reason to mess with someones food...I have worked with Chefs who have and I thought it was disgusting. The kind of thing were if someone will ask for something spicy, the Chef will say "Oh yeah? Here you go!!!" and make it CRAZY spicy out of spite...Not cool..I have seen that movie several times. That really happens.

And I personally didn't but I saw a lot of stuff in the 2 years I worked there.
I couldn't eat at restaurants for a long time after working there.
I spent 10 years in an Applebee's kitchen and I saw it happen a few times. Like LongtimeRams said, it shouldn't happen, but the truth is it does. I don't think it's widespread or anything, but it happens. Most of the instances I encountered happened when a table would walk in at closing time and the kitchen was just about clean and the cooks had one foot out the door. They would get so pissed.
I worked in other casual dining spots and 2 or 3 fine dining establishments and I would say that it is rare in the more upscale places. The chefs/cooks in those places take pride in what they are doing, as opposed to a minimum wage cook just there to get a paycheck.

One of the less thought about consequences of eating rare ground beef is the fat content of the beef. Eating a rare burger with a high fat ratio can cause you to shit your guts out violently which would be better than contracting something but still, diarrhea usually strikes at the worst time.Can you eat a rare burger (safely)?
Burger- Medium (but I can and will eat medium well or even well done, but it's not preferred)
Steak- Medium (I will not eat any other type of steak, so don't even bother)

Burgers....I always order medium unless I'm doing the cooking and then I'll go with medium rare.
I have a hard time getting y burgers to come out medium rare, they seem like they go from raw to well done in under 60 seconds with nothing in between.
Build a bigger burger brah! Make em thicker.
