She dies and he stays in America. Not sure about where to place the kids but I think that the girl will end up being a spy and the boy ends up playing hockey for the American National team.

Yes, Andrea has to be a spy. I don't see why they would bring a somewhat known actress to play that part if it's just fluff. I think it's been a few years in actual time that she's been with Stan, so it's a pretty long con.

I don't see why the smoking was such a big revelation in thinking Elizabeth is the spy. A lot of people smoke.

I don’t know that it was a huge revelation, it was just yet another clue that was not able to rule them out. Once he got the idea, he has been trying to rule them out but couldn’t.
