Don Beaupre of the Capitals on Nov 24, 1989 vs Pittsburgh might have allowed 4 goals on 4 shots.

From reddit:

Only 13 goalies had given up three goals on three shots since the NHL started keeping stats about shots on goal in the late 80s

Braden Holtby‘s 0-percent saving performance against the Colorado Avalanche on Monday night puts him in ignoble company. Since the NHL started keeping stats about shots on goal in the late 80s, just 12 goalies had given up three goals on three shots. Holtby just became the 13th.
It hadn’t happened in two years and had only happened twice since the 2005 lockout.

Only one goalie in NHL history has been scored on four times on four shots. It was Washington’s own Don Beaupre, who got pulled after 13 minutes in a 1989 post-Thanksgiving game against the Pittsburgh Penguins.

That list would not have been complete without the Sieve.

Really wish Irbe had done that vs. Toronto in May 2002 instead of March 1999.

We used to serenade a certain Devils goalie with "It's a long way to Chris Terreri, it's a long way to go"
