Oh ok, I've seen the judgment call on stuff where like a fan interferes in foul territory or the ball gets stuck under the wall, but never on a ground rule double.

It's really a misnomer to call it a "Ground Rule Double," because it's not a rule. I've seen three bases awarded in the past, for runners going on the pitch, and such. It's rare though.
