Under the current format no division has had all its teams make the playoffs, so that answer is no. Under the old four division and divisional playoffs, the fifth team in each division never made the playoffs, so there goes that. After expansion in 67, it was possible, but probably didn't happen. Maybe back in the original six days it did.
