Yes, Clapton wrote the song about his love for her while she was married to Harrison.

She eventually left Harrison and married Clapton, whom she divorced some years later.

Harrison asked for the divorce (partly)because of her affair with Ron Wood!

She appears that she was a total groupie, albeit at a high level.
