A pitcher has to control the inside of the plate or he gets rocked. I have no issue with hitting batters in the waist/hip area but those who target the head should be tossed without warning. Yes a ball can get away but you can tell the "Oops my bad" from the "Oh Yeah take this" 9 out of 10 times.
Faking a tag is a good way to get ear holed as well.
