The situation is reminiscent of Bruce Lee's son Brandon Lee getting killed in 1993 on the set of The Crow.

Massee fired the improperly prepared prop gun that killed Lee on the set of “The Crow” in 1993. Lee, the son of martial arts star Bruce Lee, died in surgery on March 31, 1993. Than you have blanks ..Cartridge and gun powder but no bullet

The bullet from the dummy round dislodged into the gun barrel in once scene ...that that same gun was loaded with blanks for another scene.

Yep, Somebody broke the protocols that were put into place after the Brandon Lee incident.

I would venture to say that there are several people that broke protocols. From what I am reading they were live rounds loaded into the gun and believe that everyone from the props department to the actor must verify the rounds used were not live rounds. I could be wrong but if I am then I see that being a new standard going forward.

How & why would live rounds ever be needed on a movie set. And even if they were ..you would think there would be ridiculous checks & measures.

I do find it hard to believe that Baldwin was just flat out handed a loaded gun by the people who are trained to prevent that very thing.
I think Baldwin may very well be one of several people who broke protocol rules.

they use real guns loaded with blanks to get realistic effects during filming apparently one or more of the blanks were defective.

If you put a full load blank into a plugged gun ...it would blow up in your face when fired.
