There are a couple of factors which jump out in defense of the MLB: the strength of the MLBPA during those years and the rudimentary at best nature of testing at the time. To be honest, testing is still ineffective, because the developers of steroids are always one step ahead of the tests, and they always will be.
Perhaps they weren't proactive enough, and perhaps that was intentional, but at the same time, I'm not sure their efforts would have gotten them very far anyway.
