/He's racking up penalty minutes ala Randy Holt this period.

I think this rule has been around for a few years now but I can't remember another specific incident when it has been called. Does it only apply if the other player is not wearing a visor?

I think if you instigate a fight, you need to make sure you lose your helmet first if there is a visor attached. Smid fought Nolan (who also wears a visor).
