i remember the Astro's did it a couple years back with Kaz Matsui and Bourn(i think it was Bourn at least)

Astros actually did it this year, in the first inning, against the Dodgers. Mattingly didn't appeal and teh Astros reverted back to regular order next time no problem. I'm sure Don heard about his mistake to not appeal, so he didn't make that mistake again.

You know, you are right. I tired looking up where I saw it, finally came across this:

The Astros bat out of order in the first inning, as they do not follow the lineup card submitted to the plate umpire prior to the game. Dodgers manager Don Mattingly does not appeal the batting out of order since no runner reached base. Astros manager Bo Porter can correct the mistake and have his team bat in proper order the remainder of the game without penalty. True or false?

So, must be a typo or a made up scenario because Bo Porter has only been the Astros manager for this year, and the dodgers haven't played them yet.

I remember a while back the Indians had Manny Ramirez and another Ramirez and they accidentally batted out of order, so the pitcher was forced to bat in the line up the rest of the game (aka NL style).
