With all the rules about hits to the head and such, get your players to have loose helmets, takes a hit, helmet comes off, ref sees the helmet come off and assumes it must have been from an illegal hit and throws the flag.
Don't think it's ever worked but I could see some players and teams trying that just to gain an advantage in a chicken shit way.
