IST was a massive success for the NBA.
Giving a team a play in spot isn't exactly going to stir things up too much.
The reason for the IST was to make the Nov/Dec games more meaningful. And it's been an astounding success.
But sooner or later people will stop caring. Especially the fans.

They already messed with the seeding with the play-in games, so I don't think it's a big addition either. Ultimately, I doubt many IST winners will be out of the playoff picture anyway, barring big injuries midseason.

No exactly. It'll probably be years if not decades before a non-playoff type team makes any kind of noise in the tournament.
So what harm does it do to offer a Play In spot?
We're not just talking about games played 1/4 of the way in. We're talking about winning a tournament.
Only winning $500k doesn't mean much to players making $40m a season. Nor does it matter to the fans.
If they want a IST then make it meaningful.

I read that Lebron was really into winning it and I read this morning that the Lakers young guys were begging to win it. I forget who it was but for 1 of the Lakers young guys, that $500k was over 40% of his salary.
The games were fun and most had a playoff like atmosphere. The Pacers players were saying it was good for them for that reason.
So, it seems like the players were plenty motivated.

Of course players making the league minimum would want that extra $500k. But they're not exactly going to win it by themselves are they?
Now picture a team like the Bulls who are currently 12th in the East. Lavine is making, what, $40m this year. Think he really cares about that? Or would there be a lot more motivation to play well if a Play In spot was on the line.
What's his motivation for the rest of the season if they have a playoff spot secured in December?
Stupid and unnecessary are reasons.
