I think the without a helmet part is a little off. It's a penalty to take your helmet off while you're on the field. This was different than Harbaugh, and he didn't remove his helmet while on the field. He simply went out on the field during a dead ball today. Last week, I can see how it could've been a penalty but I also can see why the refs wouldn't want to penalize someone for that. that's just my opinion though.

The ball hadn't been respotted after a replay review. The fact that he started standing on the sideline as opposed to the field is irrelevant.

The play was stopped so it didn't matter if he had a helmet or not it didn't affect the field of play.
