If you've ever bought wine and/or liquor, you are probably aware that they often offer rebates on purchases.
The instructions are written in type-face so tiny, I have to use a magnifying glass. The standard requirements are always:

...legible
...complete
Then they provide space to write things like an email address that can't physically be done within the above requirements.
