Steve Kerr and Steve Novak did it. I didn't even have to add the filter for the 50% season, because the 50% 3P% and 90% FT automatically removed sub .50% FG players.

Novak only attempted 8 FTs though, so I should remove him.

So really only Kerr has done it.

If I remove the requirement of at least 40 3-point attempts, then Darrell Armstrong did it, also in the same season as Kerr.
