When I trained for a root cause analysis team this was the incident we used. It was a tragedy not just because of the lives lost or money invested but in the political pressure to violate safety standards and willingness of a contractor to do so hoping for another contract. The launch should have never happened.

Nope. Although he was the LSU coach when the filming of "The Blind Side" took place, he was still the coach at Oklahoma St. when the actual events depicted in the movie took place.
